Book an appointment?

Direct Access Appointments - an appointment with a GP, Nurse Practitioner or Minor Illness Nurse. Patients will be asked to give a brief explanation to the Clinical Receptionist and allocated an appointment accordingly. Patients are encouraged to contact the surgery as early as possible for a morning appointment or visit and early afternoon for a p.m. appointment.

Pre-bookable Appointments - GP routine appointments can be booked up to 4 weeks in advance.

Telephone Consultations - an allocated time is given to the patient for the GP to contact them to discuss medication reviews, test results, or anything which does not require a physical examination.

Nurse Advice Line - Patients contacting the surgery requiring non urgent clinical advice will be asked to give brief details to the Clinical Receptionist. A member of the Nursing Team will return the patients call in due course.

Telephone Consultations

Many people find it more convenient to discuss an issue with their doctor over the phone rather than taking time to come to surgery. The sort of things which seem suited to telephone consultations are:

  • Discussing test results;
  • Discussing issues about new medication;
  • Discussing outcomes of hospital appointments.

Problems which require a doctor to examine you are, obviously, not suitable. Minor Ailments can still be discussed with the Triage Nurse.

To arrange a telephone consultation, simply ask a receptionist to book it with the doctor of your choice. A mutually convenient day and time will be booked. It is important that you leave a note of the telephone number where you can be reached at the booked time and any extension number needed.

If you have any doubts about whether you should book a telephone appointment ask the receptionist for advice, or, alternatively, your doctor at your next appointment.

Note: You should note if your telephone is set not to accept “number withheld” calls then we will not be able to call you.

Register?

To register, please pick up a patient registration pack from reception. These forms are also available online, below. An online service is coming soon. If you have any queries, any member of our staff will be happy to help you either at the surgeries or on the telephone.

Adult Registration - Please can you complete the following and return to the receptionist.

  1. New patient application form
  2. Alcohol Screening and Smoking Questionnaire
  3. GMS1 Registration Form (purple form)
  4. Ethnic Origin Form
  5. Changes to your Records - Please read

Child Registration - Please can you complete the following and return to the receptionist.

  1. GMS1 Registration Form
  2. Ethnic Origin Form

Request an ambulance?

If you require an emergency ambulance, dial 999.

Otherwise, you should ring the surgery on 0844 477 8787. It should be noted that we require the following information:

  • The Hospital that you are going to
  • The Consultant / Doctor that you are seeing
  • The Date and Time of your appointment
  • Whether or not you are an In Patient or an Out Patient
  • Whether or nor not you require a car or and Ambulance
  • Whether or not you require an Escort
Obtain test results?

Please telephone after 11.00am for results of smear tests, X-rays or laboratory tests. Information cannot be given to another person unless we already have your express permission.

The practice will inform patients of any abnormal results that need to be acted upon.

Complete a form?

The doctor is able to sign and complete claims and forms on your behalf. However, this may have a charge associated with it. The reception team know the costs involved, so you can ask them or phone us. Many of these charges can often be claimed back from your insurance company or employer.

Examples of these forms are:

  • Private Sick Notes
  • Holiday Cancellation
  • Private Letters
  • Insurance Claims
  • Sickness / Accident Insurance Benefit Claims
  • Driving License Photographs
  • Freedom from Infection

Please leave these forms with the receptionist, who will pass them on to the doctor

NB: We are no longer able to complete Passport forms. All forms submitted will be sent straight back.
